Golden Buckeye Card

A Golden Buckeye Card is a free state program card for residents age 60 and older and those 18-59 who are disabled that offers discounts at participating businesses across Ohio, including pharmacies and retail stores. 

If you recently turned 60, are an eligible adult with a disability, or need a replacement/updated card, fill out the form online or visit the Library in person. Staff members must verify proof of age or disability in accordance with the instructions on the registration form.

Tax Forms 

During tax season, the Library has a basic selection of federal, state, and local income tax forms. Patrons may be charged a printing fee for additional forms.

The Library provides Voter Registration and Absentee Ballot Request forms at our Main Desk.
